Statistical Analysis for Health Psychology Research

Health psychology statistical analysis is the statistical design, analysis and reporting of studies of how beliefs, emotions and behavior relate to health and how they can be changed, using self-report scales, behavioral logs, diary or device measures and patient-reported outcomes. The data come from questionnaires with missing items, from outcomes recorded many times per person, and from surveys in which a belief and a behavior were measured at the same moment. A workflow that sums scales without scoring rules, fits a mediation model to one time point and describes it causally, or compares repeated measures with independent-groups tests misreports both the effect and its uncertainty. OnlineSPSS, a specialist statistical consultancy for academic, clinical and professional research, provides health behavior data analysis through in-house PhD-level statisticians, who settle the scoring rules, the time order of the measures and the model for repeated outcomes with you before testing begins.

WHEN TO ENGAGE

Statistical Support at Different Stages of Health Psychology Research

Researchers usually contact us at a recognizable point, from the first plan for a behavior change study to a reviewer's question about a mediation model.

Designing a Behavior Study You are planning a behavior change trial, a diary study or a longitudinal survey and need the measurement schedule, the primary outcome, the timing of the mediator and the sample the design can support settled.

Analyzing Repeated Outcomes Self-report and behavior measures are collected at several time points, and the longitudinal model, the scoring of each scale and the handling of missed assessments need to be chosen, run and checked.

Testing a Mediation Pathway A model of how a program or a belief is linked to behavior is planned or already run, and its time order, assumptions and wording need to be fitted to the design you actually have.

Responding to Reviewers A reviewer or advisor has asked about the validity of a scale, the handling of missing items or the causal language used for a survey model, and the analysis and the text need to agree.

RESEARCH CONTEXT

Research Designs and Data We Work With in Health Psychology

These datasets describe beliefs and behavior around health, and each has a structure that decides the model. Evaluations of allied-health practice and clinical services belong with Health Sciences.

01
Behavior Change Trials

Randomized and pragmatic trials of programs for activity, diet, smoking or adherence, with behavior measured by self-report or device at several follow-ups.

02
Patient-Reported Outcome Studies

Symptom, quality-of-life and coping scales built from many items, with missing responses and a meaning that depends on the scoring rule.

03
Longitudinal Cohort Surveys

Waves of questionnaires on health beliefs and behavior, with dropout that is related to health and a mediator and outcome sometimes measured together.

04
Diary and Intensive Longitudinal Data

Daily reports paired with wearable data, giving many correlated measures per person, device days with no wear and prompts that were never answered.

05
Surveys of Health Cognitions

Single-occasion surveys of intentions, risk perception or self-efficacy, often analyzed with models that ask more than one time point can show.

06
Brief Intervention Pilot Studies

Feasibility and pilot studies with modest samples and uncertain effect sizes, where estimates need to be reported with their uncertainty.

METHODOLOGICAL CONSIDERATIONS

Statistical Issues We Consider in Health Psychology Research

Before any pathway or effect is interpreted, your time order, your scale scoring and your missing data are examined, because each can alter a conclusion more than the model does.

Randomized Program, Unrandomized Mediator Randomizing a program does not randomize the mediator, so the mediator-outcome link can still be confounded. The assumption is stated, and a sensitivity analysis shows how strong a confounder would have to be to explain away the indirect effect.

Scoring and Missing Items Scoring rules, such as how many items may be missing before a scale score is dropped, are fixed beforehand and applied consistently, and reliability and validity are checked in your own sample.

Repeated Measures per Person Outcomes recorded many times per person are modeled with a longitudinal model in which each person has their own level and rate of change, not with repeated independent tests at each time point.

Attrition Related to Health People in poorer health or with weaker motivation leave a study more often. The pattern is examined, and the missing-data assumption behind a mixed model or imputation is tested in sensitivity analyses.

Behavior as an Outcome Self-reported behavior often differs from device-recorded behavior and comes as counts or bounded scores. The distribution decides the model, and the two sources are not pooled before their agreement is checked.

Change Patients Would Notice Change in a patient-reported outcome is read against the smallest change that matters to patients, and the confidence interval is reported beside the estimate, not the p-value alone.

STATISTICAL METHODS

Methods Selected for the Research Question and Study Design

Each method family follows the outcome and the time structure of your study, and the line under each title says why it suits health behavior data.

Regression & Predictive Modeling

Mediation and moderation models describe how a belief or program relates to behavior, with indirect effects given with intervals and worded as associations unless the design supports more.

Explore Regression & Predictive Modeling
Longitudinal & Multilevel Analysis

Repeated behavior and symptom measures are modeled so that each person has a trajectory, and cross-lagged models estimate the associations between waves without claiming direction.

Explore Longitudinal & Multilevel Analysis
Multivariate, Scale & Reliability

Patient-reported scales are checked for reliability and dimensionality in your sample, and missing items are handled by a rule that was fixed before the scores were computed.

Explore Multivariate, Scale & Reliability
Structural & Latent Variable Modeling

When a health belief is measured by several items, latent variable models separate the construct from its measurement error before the paths between constructs are estimated.

Explore Structural & Latent Variable Modeling
Comparative & Hypothesis Testing

Pilot and feasibility studies compare conditions as a difference with its interval, with the baseline score as a covariate or a paired test where the design is within-person.

Explore Comparative & Hypothesis Testing
Bayesian & Computational Methods

Indirect effects receive bootstrap intervals, and diary entries or questionnaire waves that were missed are handled by imputation, with sensitivity analyses for the missing-data assumption.

DELIVERABLES

Statistical Outputs Prepared for Your Research

The outputs follow the structure of a health behavior study, from the review of scoring manuals to the files that regenerate every table.

Data & Study Review Questionnaires, scoring manuals, the measurement schedule and the intended order of exposure, mediator and outcome are reviewed before any model is fitted.

Statistical Analysis The agreed longitudinal, mediation or measurement models, with convergence and residual diagnostics, the missing-data analyses and the planned sensitivity analyses.

Effect Estimates & Uncertainty Effects on behavior and symptoms with confidence intervals, indirect effects with bootstrap intervals, reliability coefficients with intervals and a minimal important change where one is defined.

Tables & Figures Participant flow, scale and score tables, mediation path diagrams, trajectory plots and the numbers of completed assessments at each wave.

Statistical Interpretation Results are explained for behavior change and patient experience, associations are worded as associations unless the design supports more, and limits are stated for the discussion.

Analysis Files SPSS syntax, R scripts, Stata do-files or Mplus input files, with the scoring rules documented so that every table can be regenerated by another analyst.

REPORTING

Reporting Health Psychology Research

The standard depends on the design of each study you report, and each is applied only where it fits.

Quantitative articles are prepared to APA JARS-Quant. Randomized behavioral trials report to CONSORT 2025, and the program itself is described to TIDieR so that another team could deliver it. Cohort and cross-sectional surveys report to STROBE. A mediation analysis can also follow AGReMA, which asks that the causal assumptions, the time order of the measures and the sensitivity analyses be stated. The scoring rule for each scale, the handling of missing items and the participant numbers at every wave are tabulated from the analysis itself, so the text and the output agree, and your target journal’s instructions are checked last.

ILLUSTRATIVE EXAMPLE

How the Analysis May Look in Practice

An illustration of how a behavior change trial with a mediation question moves from a design to a defensible, reportable result.

ILLUSTRATIVE EXAMPLE · WORKSITE PHYSICAL ACTIVITY PROGRAM RESEARCH · UNITED STATES

A worksite wellness team tested a randomized text-message program for physical activity, and asked whether self-efficacy carried part of its effect, with activity recorded by a wearable device and a questionnaire.

The first plan measured self-efficacy and activity at the same final follow-up, which would not have shown that self-efficacy changed first. Before outcomes were examined, we moved the self-efficacy measure to the first follow-up, kept activity as the last, and fixed the analysis in writing: a mixed model for weekly activity with baseline adjustment, and an indirect effect with a bootstrap interval, worded as an association because the link from self-efficacy to activity was not randomized. The scoring rule for missing questionnaire items was set in advance, and days without wearable data were handled by multiple imputation, with complete cases as a check.

Outcome: The team received the program effect and the indirect effect with 95% confidence intervals, the sensitivity analysis for unmeasured confounding of the mediator, CONSORT 2025 flow counts, a methods paragraph and the scripts.

FREQUENTLY ASKED QUESTIONS

Questions About Statistical Analysis for Health Psychology

Short answers to what researchers ask before sharing a behavior study, from mediation and missing items to repeated measures, confidentiality and the assessment.

Health psychology most often uses longitudinal mixed models for repeated outcomes, mediation and moderation analysis, scale reliability and factor analysis, and group comparisons reported with effect sizes and confidence intervals. The choice depends on how many times each person was measured and on the order in which exposure, mediator and outcome were recorded. Our Statistical Methods pages show how each family is applied.

A mediation model can be fitted to one time point, but it cannot show that the mediator changed before the outcome, so the result describes an association pattern and not a causal pathway. Where only the exposure was randomized, its effect on the mediator is better identified, yet the mediator-outcome link stays open to confounding. We state the assumptions and run sensitivity analyses, and a design that measures the mediator first is the stronger answer.

The scale developer’s scoring rule is applied where one exists, such as computing a score only when enough items are answered, and the rule is fixed before outcomes are examined. Reliability is reported for your sample. Participants with a whole assessment missing are handled by a model that matches the missing-data assumption, and sensitivity analyses show how much the conclusion depends on it.

Days or occasions are treated as nested within persons in a multilevel model, which separates the change within a person from the difference between persons. Time-varying predictors are centered on each person’s own mean so that the two effects are not blended, and autocorrelation is modeled when occasions are close in time.

Study datasets, protocols and analysis files are handled confidentially, and project information is not published. An NDA is available where required and can be arranged before sensitive materials are shared. Read more about confidentiality and data security.

You share your research question, design and relevant materials, and a statistically trained Research Coordinator reviews the request and responds the same day.
