METHOD OVERVIEW
What Is Fisher's Exact Test?
Fisher’s exact test assesses the association between two categorical variables in a contingency table by calculating the exact probability of the observed table, and of tables as or more extreme, given the row and column totals. Because it conditions on those margins and uses the hypergeometric distribution, it needs no large-sample approximation and suits small or sparse counts. For a 2×2 table it yields a conditional odds ratio with an exact 95% confidence interval, and the Freeman-Halton extension covers larger tables. OnlineSPSS, a specialist statistical consultancy for academic, clinical and professional research, provides Fisher’s exact test analysis within your study design, with the relevant checks, effect estimates, interpretation and publication-ready reporting.
WHEN TO ENGAGE
When Is Fisher's Exact Test Appropriate?
As an exact test for contingency tables, Fisher's test fits an association question when counts are small or sparse, and these are the research situations in which researchers most often ask us to apply or confirm it.
Small Pilot or Rare Events A pilot study or a rare outcome leaves few participants in some cells, such as zero or one event in one arm, so a large-sample approximation would not be dependable.
Sparse Expected Counts A chi-square test returned a warning about small expected counts, and the choice of an exact test, its p-value definition and its reporting must be decided and documented.
Two Groups on a Binary Outcome Two independent groups are compared on a yes or no outcome, and the result is reported as an exact p-value with an odds ratio and interval, not as significance alone.
Reviewing a Completed Analysis A Fisher's result was reported without its p-value definition or odds ratio, and a reviewer has asked. Independent Statistical Review checks the test and its reporting.
ANALYTICAL APPROACH
How We Apply Fisher's Exact Test
Within the family of Comparative & Hypothesis Testing, an exact test is chosen only after the design, the counts and the reporting needs have been examined.
Structure and Independence Each participant must fall in one cell, with no pairing or clustering. The test conditions on the margins, which the design need not fix, and matched data call for a different method.
Sparse Counts and Table Size The test has no expected-count requirement and works for any table size. Large tables can be computationally heavy, so we use an efficient exact algorithm or a Monte Carlo p-value where needed.
Two-Sided P-Value and Conservatism Software defines the two-sided p-value in different ways. The test is also conservative because the distribution is discrete. We state the definition used and consider a mid-p value.
Magnitude and Uncertainty We report the conditional odds ratio with its exact 95% confidence interval, plus a risk difference or relative risk where useful, and state how zero cells affect each estimate.
DELIVERABLES
What You Receive
As part of our Quantitative Data Analysis service, the exact analysis is delivered with the estimates, checks and files needed to report and defend it.
Data & Variable Review Each record is traced to one participant, category coding and missing responses are checked, and zero or near-zero cells are identified from the raw data before any test is run.
Method-Specific Checks Table structure, sparse cells and the design are examined, with the p-value definition documented and a mid-p or Monte Carlo sensitivity result where the choice could change the conclusion.
Statistical Results The observed table, the exact p-value, the conditional odds ratio with its exact 95% confidence interval, and a risk difference or relative risk with an interval where suitable.
Publication-Ready Tables & Figures Contingency tables with counts and percentages, and plots of proportions with confidence intervals, formatted to APA 7, CONSORT or STROBE, or the journal's requirements.
Interpretation The result is described in terms of the estimate and its uncertainty, within what the design and a small sample support, with zero cells and imprecision stated plainly.
Reproducible Analysis Files SPSS syntax, R scripts, SAS programs or Stata do-files are provided where applicable, so the exact p-value and interval can be traced and rerun if the data change.
ALTERNATIVE METHODS
When Another Method May Be More Appropriate
We choose the method by design and outcome type. An exact test suits sparse tables, and these methods fit larger samples, paired data and adjusted questions.
01
Chi-Square Test
With adequate expected counts in a larger table, the chi-square test gives a close approximation and supports residual and effect size reporting across many categories.
02
McNemar's Test
When each participant is assessed twice on a binary outcome, or cases are matched to controls, the pair is the unit of analysis and only pairs that change category carry information.
03
Logistic Regression
When several covariates must be accounted for and events are numerous enough to support them, a regression model gives adjusted odds ratios that an exact test on one table cannot provide.
04
Firth's Logistic Regression
When adjustment is needed but events are few or a cell is empty, penalized likelihood reduces small-sample bias and gives finite estimates under separation.
CASE EXAMPLE
How We Work in Practice
An illustration of how a small-count comparison moves from a planned test to a reportable, defensible result.
ILLUSTRATIVE EXAMPLE · SURGICAL OUTCOMES RESEARCH · UNITED STATES
A hospital research team compared a postoperative complication between two surgical techniques in a small single-center series and needed a test that would hold up to review.
Co-investigators expected a chi-square test, but with complications likely to be rare in a small series, the approximation would not be dependable, so we recommended Fisher’s exact test in the analysis plan and documented the reason. When the data were analyzed, one technique had no complications, so one cell of the 2×2 table was empty and the odds ratio estimate lay at its boundary. We stated which two-sided p-value the software reported, and reported the exact 95% confidence interval for the odds ratio, open on one side because of the empty cell, alongside the risk difference with its interval. Because techniques were not randomized and the series was small, we presented the result as an imprecise, unadjusted association between technique and complications.
Outcome: The team received the contingency table, the exact p-value, the exact 95% confidence interval for the odds ratio and the risk difference with its interval, a methods paragraph justifying the exact test, APA-formatted tables and the analysis syntax.

RESEARCHER VOICE
What Researchers Say
Feedback from a researcher who needed the choice of test reviewed and the output interpreted for reporting.
“We knew Fisher's Exact Test was probably the right direction, but a relatively small sample and unstable estimates was where we were stuck. OnlineSPSS reviewed the data and model rather than just rerunning the software output. The final output was concise, clearly interpreted, and ready to use in our research reporting.”

FREQUENTLY ASKED QUESTIONS
Questions About Fisher's Exact Test
Short answers to what researchers ask before an exact analysis, from table size and fixed margins to confidentiality and the assessment process.
Is Fisher's exact test only for 2x2 tables?
No, the exact approach extends to tables of any size through the Freeman-Halton extension, which software often simply labels Fisher’s exact test. Only the 2×2 case gives a single odds ratio with an exact confidence interval. For large tables with many cells the exact calculation can be slow, so a Monte Carlo p-value may be used and its settings reported.
Should I use Fisher's exact test or an unconditional test such as Barnard's?
Fisher’s test conditions on both margins, which keeps the null distribution exact even when the totals were not fixed by the design, and this conditional approach is widely accepted. Unconditional tests such as Barnard’s or Boschloo’s are often more powerful in 2×2 tables but less familiar to reviewers and less widely available. We choose before the outcomes are examined and state the approach and reason in the methods.
Which two-sided p-value is reported, and is the test too conservative?
There is more than one definition. A common one sums the probabilities of all tables no more likely than the observed one, while another doubles the smaller tail, and they can differ in small tables. Because the distribution is discrete, the test is conservative and its actual error rate does not exceed, and can fall well below, the nominal level. We state the definition used and consider a mid-p value as a sensitivity result.
Can you review an analysis that has already been completed?
Yes, we can review a completed Fisher’s exact analysis before submission or a response to reviewers. We check the choice of test, the p-value definition, the handling of zero cells, the odds ratio and interval, and the reporting, and confirm what stands or needs to change.
How do you protect my dataset and research confidentiality?
Your dataset, output files and any protocol or analysis plan are handled confidentially, and project information is not published. An NDA is available where required and can be arranged before sensitive materials are shared. Read more about confidentiality and data security.
What happens when I request an assessment, and how long does it take?
You share your research question, design and relevant materials, and a statistically trained Research Coordinator reviews the request and responds the same day. The analysis, deliverables, timeline and one fixed project fee are then confirmed before any work begins, so the timeline follows from the agreed scope.
