Clinical Study Biostatistics
The Endpoint, the Estimand and the Report
Clinical endpoint analysis built on the primary endpoint, the estimand it targets, the analysis populations and the multiplicity rules, set out in a plan and carried through to the tables of a trial or clinical study report that a reviewer can check against the protocol.

What Does Endpoint-Driven Biostatistics Involve?
Clinical study biostatistics is organized around the primary endpoint: how it is measured, whether it is continuous, binary, a count or a time to event, how secondary endpoints are ordered behind it, and which summary measure answers the clinical question. Biostatistical endpoint analysis then fits the model the endpoint form requires and controls the error rate across the endpoints that carry a confirmatory claim. Clinical biostatistics reporting presents each result with its interval and the numbers analyzed. At OnlineSPSS, a specialist statistical consultancy for academic, clinical and professional research, endpoint analyses are carried out by in-house PhD-level statisticians and documented against the protocol. The clinical question and its conclusions belong to the study team.
WHEN TO ENGAGE
Clinical Studies Awaiting an Endpoint Plan
The four situations share one question: what is the primary comparison, and was it fixed before the data were seen?
Protocol Still Open The protocol names an outcome but not the population, the handling of treatment changes or the summary measure, so the endpoint has to be fixed as an estimand while the design can still change.
Composite Under Debate Investigators disagree about which events belong in a composite endpoint or whether a responder threshold should replace a continuous score, and the choice must be settled while the protocol can still change.
Database Locked, Report Due The primary analysis is to be run and reported, with participant flow, the main result and its interval, and the safety summaries that a clinical study report needs.
Reviewer Queries an Endpoint A coauthor or reviewer asks why a population was excluded, why a composite was used or how missing outcomes were handled, and the analysis has to be checked against the estimand.
ENDPOINT WORK
How the Endpoint Shapes Each Analytic Step
The primary endpoint fixes the population, the comparison and the summary measure, and the later steps follow from those choices.
Endpoint Measure and Timing
The primary endpoint is fixed by what is measured, when, and in which form, whether continuous, binary, a count or a time to event, with the estimand it serves named in ICH E9(R1) terms.
Safety Population and Coding
Safety tables cover every participant who received study treatment, with adverse events coded to a standard dictionary such as MedDRA and summarized by system organ class and preferred term.
Composite and Time-to-Event Forms
An endpoint that is the time to a first event uses Survival Analysis, and the composite endpoint definition names every component and reports each one, so that one frequent part cannot hide the rest.
Multiplicity Control
Secondary endpoints are tested in a prespecified order, such as a fixed sequence or a gatekeeping scheme, so that the familywise error rate holds, and all other endpoints are labeled exploratory.
Gaps in the Primary Outcome
The primary analysis states its assumption about missing outcomes, and sensitivity analyses show how far the result moves if participants who left would have fared differently.
Reporting Layer
Results carry participant flow and an interval for every endpoint, following the conventions of Clinical Trials research and the guideline for the design.
ENDPOINT PITFALLS
Endpoint Choices That Undermine a Trial Result
These errors are usually decided in the protocol or the plan, long before the final model is run, so each is easiest to prevent there.
Responders by Dichotomy A continuous endpoint is split into responders at a cutoff chosen after the data were seen, which discards information and can favor one arm by construction.
Change From Baseline by Default Change from baseline is analyzed without adjusting for the baseline value, so a chance imbalance between arms at baseline can shift the treatment comparison through regression toward the mean, and precision is lost as well.
Composite Led by One Part A composite endpoint improves because its most frequent and least serious component did, and the result is read as benefit on the events that matter most to patients.
Margin Without a Basis A non-inferiority margin is chosen for convenience and not justified from the effect of the active comparator and a clinical judgment, so the conclusion is hard to defend.
Events Classified Without Blinding In an open-label trial, events are classified by investigators who know the assigned arm, so borderline cases can be counted unevenly between arms, a risk that blinded adjudication against fixed definitions reduces.
Safety Counted Unevenly Adverse events are tallied as raw counts although follow-up differs between arms, so exposure time is ignored and the rates cannot be compared fairly.
DELIVERABLES
Endpoint Plan, Analysis and Report Tables
Every item can be set beside the protocol, so that a reviewer can see which decision produced which number.
Endpoint Specification Each primary and key secondary endpoint with its measurement, time point, form and summary measure, and the rule for any responder or composite definition, ready to place in the protocol.
Testing Order Rationale The order in which key secondary endpoints are tested, the error-rate control it gives and the label each remaining endpoint carries, set out so a reviewer can follow every confirmatory claim.
Participant Flow Summary Numbers screened, randomized, treated and analyzed in each arm, with the reasons for exclusion, laid out for the flow diagram of the report.
Primary Result Tables The primary endpoint effect with its 95% confidence interval and the number analyzed in each arm, followed by key secondary endpoints in the testing order.
Safety Summary Tables Adverse events by arm with counts, participants affected and exposure-adjusted rates where follow-up differs, with serious events listed apart.
Locked-Data Programs Programs prepared on blinded data and run once on the locked dataset, with a log of the run, so each figure in the report can be regenerated by the trial team.
ILLUSTRATIVE EXAMPLE
A Composite Endpoint Rebalanced Before Enrollment
An illustration of how a composite endpoint and its testing order were settled when one component was far more frequent than the others.
ILLUSTRATIVE EXAMPLE · PERIPHERAL ARTERY DISEASE TRIAL RESEARCH · UNITED STATES
A vascular research group planning a randomized comparison of two revascularization strategies for peripheral artery disease needed the primary endpoint and the order of secondary endpoints fixed before enrollment began.
The draft protocol combined death, major amputation, repeat revascularization and wound complications in one composite, and the in-house statistician pointed out that wound complications were far more frequent than the other events and would drive any difference between arms. The group kept death, major amputation or repeat revascularization as the primary composite, analyzed as time to the first event, and moved wound complications to the safety tables. Each component was to be reported beside the composite with its own estimate. Two key secondary endpoints were placed in a fixed testing order behind the primary comparison, and the remaining outcomes were labeled exploratory. Because follow-up would differ between participants, adverse events were planned as rates per patient-year.
Outcome: The group received the endpoint specification, the testing order rationale, table shells for the composite and each component, the planned safety tables and the programs for the locked data.

FREQUENTLY ASKED QUESTIONS
Primary Endpoint Queries Answered
Harms, non-inferiority margins and secondary endpoints each carry a rule that trial reports often bend or leave unstated.
What does clinical endpoint analysis cover?
Clinical endpoint analysis covers the statistical design and analysis of a clinical study around its primary endpoint: the estimand, the analysis populations, the models, the control of multiplicity and the reporting of results. It applies to randomized and observational studies, before or after data collection. Wider design questions, such as sample size, sit within Biostatistics.
Should adverse events be tested for significance between arms?
Usually they are described rather than tested. A trial is rarely powered for harms, so a nonsignificant difference does not show safety, and testing many event types inflates false alarms. Events are summarized by arm as participants affected and as rates per exposure time, with serious events listed individually.
How is a non-inferiority margin chosen?
The margin is set before the data are seen and rests on two things: the benefit the active comparator showed against placebo in earlier trials, and a clinical judgment about how much of that benefit may be lost. Conclusions come from the confidence interval for the difference set against the margin, and both the intention-to-treat and per-protocol sets are examined. The margin itself is a decision for the study team and its sponsor.
Can a secondary endpoint be reported as a finding if the primary result was not significant?
It can be reported, but with the right label. In a fixed-sequence scheme an endpoint is tested only after every hypothesis ahead of it has been rejected, and gatekeeping schemes apply a similar condition between families of endpoints, so after a failed primary comparison later results are described as exploratory, with estimates and intervals but no claim of a confirmed effect. The same order applies to time-to-event endpoints, whose methods sit under Clinical, Survival & Epidemiological.
How do you protect my research data and confidentiality?
Your data and project details are handled confidentially, and project information is not published. Researcher names and institutions are not used publicly without permission. An NDA is available where required and can be arranged before sensitive materials are shared. Read more about confidentiality and data security.
What happens when I request an assessment, and how long does it take?
You share your research question, design and relevant materials, such as the protocol, a draft analysis plan, the endpoint list and the dataset, and a statistically trained Research Coordinator reviews the request and responds the same day. The analysis, deliverables, timeline and one fixed project fee are then confirmed before any work begins, so the timeline follows from the agreed scope.

Your Protocol Read Against
Its Primary Endpoint
A composite that leans on its most frequent component, or a responder cutoff picked late, blurs the very comparison a trial was built to make; settling the endpoint definition comes before drafting the analysis plan.
